Montefiore Medical Center AIDS Center
The AIDS Center provides HIV primary and specialty care, sexually transmitted disease screening and treatment, HIV biomedical prevention services, Hepatitis C care, and LGBTQ care at 2 ambulatory sites, the Center for Positive Living/ID Clinic and the Oval Center, and the inpatient consult services. The sites serve approximately 4,000 patients per year at about 35,000 visits and are steadily growing.
To meet the access, medical, and biopsychosocial needs of the AIDS Center's complex populations living with or at high risk for HIV, Hepatitis C, and sexually transmitted infections, the AIDS Center leadership has committed to increasing access to comprehensive, culturally appropriate care according to best practices for destination clinics as outlined by NYC DOHMH. The Chronic Care Registered Nurse supports these efforts by providing nursing assessment and care management of high risk patients, supporting the linkage and retention in care of those diagnosed with HIV and Hepatitis C, and working to improve quality of care and quality metrics across all populations served.

St. John’s Riverside Hospital HOPE Center in Yonkers, NY is seeking a full time Nurse Manager. (Monday – Friday, 8 a.m. - 4 p.m.; occasional Saturdays, 8:30 a.m. - 12:30 p.m.)
HOPE Center is an outpatient Primary Care Center that offers primary care services as well as specialized HIV primary care, Hepatitis C care & treatment, and PrEP for HIV prevention. We are a NCQA recognized Patient Centered Medical Home (PCMH) and Trauma Informed Care practice.
We are seeking a candidate experienced in an outpatient clinic setting with individuals living with HIV, Hep C, mental health, and substance use.
This position will be responsible for the overall operation of HOPE’s medical and nursing program. Utilizing the Nursing Process in evaluating and assessing the nursing needs of patients and managing patient flow, the Nurse Manager will work as an integral part of HOPE’s interdisciplinary team, including the program’s Medical Directors, Nurse Practitioners, Social Workers, and Care Managers.
